Your first appointment

For your first visit, please arrive 10 minutes early as there is paperwork to complete. Alternatively, you can download our intake forms and bring it with you for your first visit:

What to bring to your first appointment

  • Care Card number
  • Claim number (if WSBC or ICBC)
  • Doctor's referral, if applicable
  • Extended Health Plan Card
  • Diagnostic test results (MRI, CT scan, X-ray), if available
  • Comfortable clothing (yoga/sweat pants or shorts, t-shirt)
  • Intakes forms from above if you wish to do these in advance

Location

We are located at 1256 Marine Drive, North Vancouver in the Pemberton Plaza. We are right next to Save-On Foods, and there is ample free parking.

 


Direct billing: Personal Insurance

North Vancouver Physiotherapy & Sports Clinic is pleased to offer online direct billing for physiotherapy to number of extended health carriers, see complete list in the Rates/Direct Billing Page.

Please bring your:

  • Your extended benefits card with policy number and member ID
  • Doctor's referral, if required by your policy

 


WorkSafe

Please bring your:

  • Claim Number
  • BC Carecard
  • Employer information (supervisor or HR contact info, along with company name, address, phone number)
  • Any relevant medical information

Additional forms are provided in our office. For these visits please arrive 15 minutes prior to your appointment to fill them out.

If your claim is accepted there is no user fee. WorkSafe will cover the first 6 weeks of treatment, so long as your claim has been approved for physiotherapy treatments. Your first appointment will be 1 hour in length, so please make sure you mention to the receptionist that you are booking a Worksafe appointment.

Work Safe BC Injury Claims FAQ:

What do I need to do if I was injured at work?

  1. Report your injury to your employer and let them know if you will be requiring time off work
  2. Report your injury to Work Safe BC:
  3. See you Physician and let them know you were injured at work as soon as possible after your injury.
  4. Please remember to bring in your provided claim number at the time of your appointment.

Is a Doctors' referral required for me to attend physiotherapy after my workplace injury?

  • Work Safe Claims –To activate your claim you need to have seen your doctor, as they have to report your injury to Work Safe BC. You are required to follow up with your doctor on a regular basis as a part of your claim. We recommend having a Doctor's referral for Physiotherapy

How much coverage will I have with WorkSafe BC and what do I need to pay?

  • Approved Work Safe claims do not require any payment by the patient and generally 6 weeks of treatment is allowed.

 


ICBC:

As of April 1st 2019, ICBC appointments will be at NO COST.

Please bring your:

  • Claim Number
  • BC Carecard
  • Adjuster information
  • Any relevant medical information

ICBC Injury Claims FAQ:

What do I need to do if I was injured in a car accident?

  • Call ICBC to set up a claim. You will receive a claim number from ICBC.
  • We recommend that you see your Doctor, and get a referral for Physiotherapy. ICBC also recommends that you follow up with your Doctor every 4 weeks.
  • Remember to bring you claim number to your first appointment.

Is a Doctors' referral required for me to attend physiotherapy after my car accident?

  • ICBC Claims – If this is a new accident and you are seeking treatment within 12 weeks of the injury, you do not require a Doctor's referral. Within the 12 weeks you are able to attend 25 physiotherapy visits. 
  • After 25 visits, namely if you need an extension for physiotherapy treatment or if your injury is more than 12 weeks old you will require a Doctor's referral.

How much coverage will I have with ICBC and what do I need to pay?

  • ICBC will pre authorize 25 physiotherapy visits
